Family Practice Physician
We are seeking a Family Practice Physician to provide primarily clinical care to our patient population. This position also includes administrative duties including implementation and operation of our Mobile Health Unit that targets a medically underserved population. Physician must have a valid NY State License and will attend at St Luke’s Hospital.
P/T Weekend Licensed Practical Nurse
We are looking for a P/T LPN to work the weekend shift at our Opioid Treatment Program. The hours are from 7am-12pm Saturday and Sunday. Nurse will be responsible for dispensing of Methadone to our clients. Applicant must have a valid NYS License.
P/T Maintenance Worker
We are seeking 2 part time maintenance workers to help maintain our new facility on 147 Lake Street. Applicant must have experience with floor waxing and buffing. Evening and Saturday hours are required.
HIV Case Manager
We are seeking a qualified Case Manager to serve our HIV client population. Duties include managing a case load of patients to ensure a quality of care. Applicant must have experience in psychosocial assessments, accessing social support services, providing community referrals and follow up to ensure continuity of care. Applicant will provide peer support and focus groups, crisis intervention, and outreach to clients. Experience with HIV pre/post test counseling a plus. APPLICANT MUST BE BILINGUAL.
Breast Feeding Peer Counselor
We are seeking a peer counselor to provide nutritional education and advocacy for breast feeding. Applicant MUST BE A PAST OR PRESENT WIC PARTICIPANT in order to qualify. Applicant must be bilingual.
Volunteer with The Greater Hudson Valley Family Health Center
We are currently looking for motivated people to volunteer with us! Assignments include patient support services and administrative services. This is an opportunity make a difference in the Hudson Valley community by supporting a health care organization devoted to proving high quality health care services to ALL of those in need.
