Your Healthcare is important to us. We accept many forms of Insurance Payments.
At the Greater Hudson Valley Family Health Center Inc. our mission is to provide comprehensive high quality health care to all who require our services, regardless of a patient’s ability to pay. However, because of the programs offered by the federal and state government and many insurance carriers, it is easier than ever to be enrolled in a health care plan for you and your family based upon qualifying factors such as income, and family status.We have also provided a partial listing of the many other insurance carriers who cover our services. Please note that the list of insurers who cover our services changes frequently, as does the nature of the services that are covered by individual insurance companies. To be sure that you are covered you should contact your member services representative at the number listed on the back of your I.D. card, and then verify coverage with our receptionist when you call to make an appointment with us. In addition, there are many locally based insurance programs, most of which are provided by local Unions or small employers which cover some of our services. If you are a participating member of one of these carriers please be sure to check with the carrier to insure that your carrier will cover the services provided by the Greater Hudson Valley Family Health Center.

Uninsured Patients

Income Verification will be required for all non-insured patients. If income verification is not provided, please be prepared to pay $120.00 dollars for the visit. Please also note that if your health care provider orders lab work, medical treatments, immunizations, PPD’s, EKG (these are just a few examples). You will be required to pay at the time these services are rendered an additional cost or you will be billed for the additional services

Sliding Fee Scale qualifications: If you intend to apply for our Sliding Fee Scale Program, you will require the assistance of a Patient Services Representative. It is recommended that you make an appointment at 563-8016 and allow yourself at least 30 minutes prior to your medical appointment time
to apply for these services.

In order to apply for sliding fee services you must bring with you documentation of all house hold income; your income and that of any individuals who reside in your home. For each we will accept any
of the following item(s):

Last 4 Pay stubs, W2 tax statements, Proof of Disability Income, Child support statements and or a letter of support, unemployment benefit letter/paystubs.

If your child is uninsured and or undocumented they will qualify for health insurance. Please stop and see one of our Patient Service Representatives who can assist you. In our main lobby multiple Managed Care Plans have representatives who you may speak with about low or no cost health insurance.
